This Release of Liability applies to activities that are NOT organized, sponsored, supervised, or endorsed by Fun Shine Child Care. Such activities may include, but are not limited to, field trips, sporting events, outings, celebrations, or other events that a parent or guardian chooses to have their child participate in outside of the daycare's regular licensed care and programming.
I understand and acknowledge that the activity described in this form is not a Fun Shine Child Care activity. Fun Shine Child Care, its owner Christiane “Miss Chris” Gurule, and its staff are not responsible for organizing, supervising, transporting, or overseeing my child during this activity in any way.
I understand that participation in this activity is completely voluntary and is undertaken at my own risk and my child's own risk. I am aware that any activity can involve risks of injury, illness, property damage, or other harm, and I knowingly and voluntarily accept all such risks on behalf of myself and my child.
In consideration of my child being permitted to participate in this activity, I hereby release, waive, and hold harmless Fun Shine Child Care, Christiane “Miss Chris” Gurule, and all Fun Shine Child Care staff and representatives from any and all claims, demands, liabilities, damages, costs, or expenses of any kind arising out of or related to my child's participation in this activity, to the fullest extent permitted by law.
I confirm that I am the parent or legal guardian of the child named in this form and that I have the legal authority to sign this release on the child's behalf. I have read this Release of Liability in full, understand its contents, and agree to its terms voluntarily.
